Job Description:
Liquid Personnel is recruiting an Adult Social Worker for its client based in Conwy.
What will your responsibilities be?
- Assess needs and create care plans for older adults in hospital or community settings.
- Coordinate safe discharges and arrange follow-up support services.
- Safeguard vulnerable adults and advocate for their rights and wellbeing.

Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
