Liquid Personnel is recruiting an Adult Social Worker to join its client’s Home First Neighbourhood team, based in Oxfordshire.

About the Role:

This opportunity is with a client in Oxfordshire who is transforming its hospital discharge services.
The aim of the service is to ensure residents have their needs assessed and met in their own homes, rather than at the hospital bedside.
Home First D2A is a developing service that encourages creative thinking, innovation and continuous improvement from staff across the team.
As part of this role, you will:

  • Work with reablement providers and the voluntary sector to support and guide people to regain independence in their own homes, using a strengths‑based approach to goal planning during short‑term intermediate care. This intervention will take place following hospital discharge or to help prevent hospital admission.
  • Use skills and knowledge in activity analysis, assistive technology, aids and minor adaptations to reduce formal care needs wherever possible. Moving and handling expertise will also be required.

About You

You will be a motivated and adaptable Social Worker, confident working in a fast‑paced, multidisciplinary environment. You will use a strengths‑based approach to help people regain independence after hospital stays and support safe, timely discharges through the D2A model.

You will complete assessments within 72 hours of discharge, applying sound professional judgement to make proportionate, person‑centred decisions. Strong organisational skills, effective prioritisation and confidence in navigating complex systems are essential.

What will your responsibilities be?

  • Carry out strengths‑based assessments within 72 hours of hospital discharge, in line with the Discharge to Assess (D2A) model, focusing on maximising independence and reducing long‑term support needs.
  • Work collaboratively with ward staff, therapy teams and wider MDT partners to support safe, effective and timely hospital discharges.
  • Undertake holistic Care Act assessments, reviews and interventions, ensuring care and support plans are person‑centred and promote wellbeing and resilience.
  • Contribute to a 7‑day rota, participating in shift patterns that ensure consistent service delivery.
  • Manage a diverse caseload independently, prioritising effectively in a fast‑paced community and hospital‑linked environment.
  • Use a strengths‑based, outcomes‑focused approach to develop support plans that build community connections, informal networks and long‑term independence.
  • Liaise with families, carers, health partners and community organisations to ensure coordinated support and effective discharge planning.
  • Apply the Mental Capacity Act and undertake mental capacity assessments where required, ensuring decisions are lawful and in the person’s best interests.
  • Identify, respond to and escalate safeguarding concerns in line with Making Safeguarding Personal and statutory requirements.
  • Provide professional advice regarding equipment, moving and handling, and home adaptation needs, working alongside OT colleagues where appropriate.
  • Maintain accurate, timely and defensible case records and reports in line with the end client’s policies.
  • Contribute to service development, reflective practice, team meetings and organisational learning.
  • Work within the end client’s competency framework and fully engage in supervision, appraisal and ongoing professional development.

Qualifications and Experience:

To be successful in this role you must have;

  • Qualified Social Worker with current Social Work England registration.
  • Strong understanding and application of strengths‑based practice within community and hospital discharge settings.
  • Ability to complete timely assessments under the D2A framework and work confidently within 7‑day working/shift patterns.
  • Experience working collaboratively with ward teams, therapy staff, community services and MDT partners.
  • Good understanding of the Care Act 2014, Mental Capacity Act 2005 and Making Safeguarding Personal.
  • Excellent communication, analytical and report‑writing skills.
  • Ability to balance risk, promote independence and support people to remain safely at home.
  • IT literacy and confidence using digital systems to record high‑quality case notes.

Desirable

  • Post‑qualifying training (e.g., BIA, AMHP, Practice Educator), or willingness to undertake further training.
  • Experience in equipment provision, moving and handling or housing adaptations.
