Social Worker - Learning Disability

What will your responsibilities be?
The successful candidate will play a key role in supporting adults with learning disabilities, with a primary focus on reassessments and statutory reviews. You will work in a collaborative environment, contributing to positive outcomes for individuals through high quality, person centred social work practice.
- Safeguard and promote the wellbeing of adults through effective assessment, care planning, and review
- Undertake assessments in line with the Social Services and Well being (Wales) Act 2014
- Identify personal outcomes and develop or review care and support plans as required
- Assess and manage risk in a balanced and proportionate way that promotes independence, choice, and co production
- Build on individual strengths to achieve positive and sustainable outcomes
- Work collaboratively with professionals and external agencies as part of a multi disciplinary approach
- Maintain accurate and up to date electronic records, including case notes, assessments, and reviews, in line with departmental policies
Additional Information:
- Community based role with travel required across the service area
- Hybrid working model with one office based day per week and flexibility for additional office attendance if preferred

Qualifications and Experience:
To be successful in this role you must have;
- Qualified Social Worker with relevant registration
- Proven experience within adult services, ideally within learning disability teams
- Strong assessment and review experience
- Ability to work independently and as part of a team
