Liquid Personnel is recruiting a Social Worker for its client's Adult D2A Hospital Discharge Team at Blackburn with Darwen
What will your responsibilities be?
- Complete strengths‑based Care Act assessments to identify needs and plan safe, timely discharges through D2A pathways.
- Work closely with hospital teams, community services, and families to coordinate person‑centred discharge plans that promote independence and recovery.
- Develop, implement, and review support plans, managing risk appropriately and ensuring adults are safeguarded throughout the discharge process.

Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
